nuts. Apparently this AC jet was on final and was lined up to land....on a taxiway containing 4 other long haul jets all carrying at least 200 people.

It wasn't until late on final approach that AC says he sees "lights" on his runway.

Then a United pilot says the AC jet went right over him as they did an emergency climb and go around.

I listened to a podcast talking about our visual blind spots and how the brain will sometimes filter out information. One of the studies they quoted was on airline pilots and how all the accidents in the simulator were from planes on the runway during landing. Apparently, the brain has trouble reconciling things it does not expect, such as the plane on the runway, and pilots will not even see it consciously (side note: motorcycles have this same effect and it's the reason why the driver of a car will say "I didn't even see him"). In any case, thank goodness he saw it with enough time to avoid the accident.
 
That should be about it for his career. How did two pilots not figure out they were headed for a taxiway? They need to update their Google maps and stop printing off mapquests.

I spoke to 20 year commercial pilot with previous RAF experience. He said SFO is one of the most dangerous airports due to the high traffic and the horrible runway layout. On the south side you have two runways taking off from west to east that cross another 2 runways that run north/south.
